iHealth is a statewide collaborative of community-based organizations united to advocate for and negotiate on behalf of our communities, our members and the chronically ill healthcare recipients we serve and to represent those programs and people within the broader healthcare system.
iHealth began in 2013 as a response to significant healthcare reforms impacting member organizations. Initial members included former case management (Legacy) programs that were transitioning to the new Health Home Model of care management. Today membership has broadened to include any community based organization (CBO) offering care management as well as organizations seeking to increase services and connectivity with managed care plans, Performing Provider Systems inside of the Delivery System Reform Incentive Payment Program (DSRIP) or other healthcare payers.
The healthcare landscape today is complex and requires collaboration among CBOs like never before. iHealth is a group of forward thinking CBO leaders looking to stay on top of current trends, gather and share information, plan their organizations future and advocate around policy and workflow issues that will impact success.
