ACCM is an approved Health Home, as well as an approved Utilization Review Agent under the NYS Department of Health (DOH), working since2013 with some of the largest HMOs and insurers in the Northeast, with emphasis on NYC.
Our team’s mission is to offer and provide coordinated, collaborative culture-specific care that raises the performance-bar among providers, and exceeds client expectations. ACCM’s relationship-based care framework, client-centric planning, and implementation of quality improvement programs are always focused on the needs and preferences of our clients. Our partners can expect their patients to have access to evidence-based practices, care and decision making that is data-driven, and a safe, highly-engaging experience. Partners can also expect to achieve improved and more efficient provision of long-term care, reductions in ED and in-patient encounters, shorter hospital stays, lower readmission rates, better integrated care-management, and a reduction in the overall cost of care.

The company is on the leading edge of chronic care management, incorporating innovative technology to better enable remote, continuous monitoring of patients’ most important health indicators. Our extensive track record in training and deploying community health workers (CHW) led ACCM to be awarded with a contract to train up to 1,000 CHWs in 2019. And all of our CHWs operate within the framework of integrated care management teams, enabled by strong linkages with providers and integration with electronic health records. The same is true of our care managers (who are all credentialed as either Registered Nurses, Social Workers and/or Licensed Practical Nurses), Certified Medical Assistants and other human services workers. Each care management team works collaboratively with community-based providers, hospitals, and home care and social service organizations to ensure that the delivery of care is seamless across providers and settings. Our goal is to enhance our members’ quality of life and to enable them to live in good health, safely and more independently.

ACCM operates under the premise that no two patients are alike. As such, we recognize and acknowledge each member as a unique individual who requires care, treatment, and service based on their assessed and expressed needs. Our principles are guided by our patient-centered culturally sensitive care delivery model. Care management team providers are familiar with the language and culture of their members.

ACCM Health Home Care Managers personally coordinate services among each member's healthcare providers, as well as among other social service agencies which the member may be benefiting from. or could benefit from. Through this practice, our team of social workers, nurses and community health workers help to obliterate the barriers which impact quality of life and contribute to unnecessary visits to the emergency room and/ or hospitalization.
