iHealth agencies are a diverse group of community based providers throughout New York State with experience, expertise and capacity to provide chronically ill individuals including Health Home members with comprehensive care management, medical care, behavioral health and social services that result in sustained connection to medical care, long-term recovery, housing stability and improved health outcomes. Care Management & Care Coordination
Since the 2012 Medicaid redesign, the DOH Health Homes Care Management (HHCM) program has been a
cornerstone of the State’s Medicaid transformation-providing person-centered, on-the-ground, integrated care
coordination for more than 185,000 high-risk adults and children (at any given time) in every district &
community in New York, supporting the Governor’s Triple Aim. With their collaterals and family members the
number of impacted New Yorkers is greater.
The HHCM programs have been associated with DOH data & outcomes showing significant Medicaid
utilization savings and cost-containment.​
Our member CBOs, Care Management Agencies &
Medical / Social Service Provider Members:

have staff in the community & on-the-ground, identifying and connecting to care individuals with the

most complex & the highest medical, behavioral health and social service needs of the Medicaid

population (almost all dually or triply diagnosed), including persons with serious & persistent mental

illness, co-occurring mental health and addiction challenges, HIV/AIDS & more.

case manage the whole person to ensure the individual is properly connected to the care they need.

successfully address--hands on--social needs impacting health, such as housing instability &

homelessness, food insecurity, lack of transportation, paralyzing legal issues, benefits acquisition & certification challenges, and more.
