A small but determined network of former case management programs came together to navigate a changing system and advocate for the chronically ill patients they served. Over a decade later, that movement has grown into a dynamic statewide collaborative of community-based organizations (CBOs), each united by a shared mission: to represent, support, and strengthen the communities at the heart of New York’s healthcare system.

Today, iHealth is more than a network—it’s a voice. Our members span the spectrum of care management providers, from legacy programs to forward-thinking CBOs looking to expand their services, influence policy, and enhance collaboration with managed care plans and healthcare systems. As the landscape evolves, so do we—proactively addressing the complexities of care delivery, policy shifts, and system reforms.
